Linguistic Validation & Independent Translation Review

Regulator-ready evidence that your clinical outcome assessment (COA) is understood, relevant and conceptually equivalent in every target language and population — from full linguistic validation of PRO, ClinRO, ObsRO and PerfO instruments to independent translation review for clinical trials, pharmacovigilance and regulatory submissions.

Understanding the difference

These are related but distinct processes. Knowing which applies to your document type ensures you receive the right service — and the right documentation for your submission.

Linguistic Validation (LV)Independent Translation Review (ITR)
For COA & PRO instruments. A structured, multi-step process — following ISPOR good practice and expected by EMA and FDA — that ensures each translated instrument is conceptually equivalent to the source and understood as intended by patients, caregivers and clinicians in the target population, supporting content validity in every new language and culture. Involves multiple independent linguists and cognitive debriefing with target-population participants.
  • PRO instruments and questionnaires
  • eCOA and ePRO materials
  • ClinRO and ObsRO instruments
  • PerfO instruments
  • Health-related quality of life (HRQoL) measures
For clinical & regulatory documents. A rigorous back-translation and bilingual review process performed by a specialist linguist with no prior involvement in the translation. Every ITR project includes a formal written review report and a signed Certificate of Translation Quality.
  • Pharmacovigilance documentation (AE narratives, ICSRs, PSURs, RMPs)
  • Clinical trial materials (ICFs, patient information sheets, CSRs)
  • Regulatory submissions (CTD modules, SmPC, PILs, labelling)
  • Medical device documentation (EU MDR / IVDR)
If your document is a patient questionnaire or COA instrument, you need LV. For everything else — ICFs, ICSRs, CTDs, PILs — ITR is the correct process. Ask us and we'll confirm.

When to engage us

Linguistic validation becomes essential whenever a COA is used outside the language, population or mode of administration in which it was first developed. Sponsors and CROs typically come to us when:

  • A COA is being introduced in a new country, language or cultural context
  • A study is extending to a new age group (for example, paediatric) or a new disease area
  • A paper instrument is moving to eCOA/ePRO and translated versions must remain equivalent
  • A regulator, ethics committee or sponsor requests evidence of comprehension and relevance
  • A rare or hard-to-reach population needs a small, proportionate cognitive debriefing design

We scope the level of evidence to your development stage and regulatory strategy — proportionate, defensible and fit for purpose.

Our process

The steps below apply to both LV and ITR projects. For full LV, cognitive debriefing (Step 4) is included. For ITR, the process concludes at Step 5 with a Certificate of Translation Quality.

  1. Scope assessmentWe review the source document, target language, intended use, and applicable regulatory framework — and confirm whether LV or ITR is the appropriate service.
  2. Independent specialist assignmentA subject-matter specialist linguist is assigned with no prior involvement in the translation.
  3. Back-translation & bilingual reviewA blind back-translation is produced and reconciled against the source. For LV projects, forward translation and reconciliation precede this step per ISPOR methodology.
  4. Cognitive debriefingLV onlyNative-speaking participants from the target population — patients and, where relevant, caregivers — are interviewed to confirm that items, instructions and response options are understood, relevant and acceptable. Sample sizes are fit for purpose, with lean designs for rare-disease and paediatric studies. Interviews are led by our LV partners at GRC Health.
  5. Formal review reportA regulator-ready report documenting item-level findings, issues identified and resolutions applied, with clear recommendations and confirmation of suitability for intended use. If an item doesn’t work in the target culture, the report says so and proposes revisions.
  6. Signed certificateA Certificate of Validation (LV) or Certificate of Translation Quality (ITR) is issued — ready for your EMA, MHRA, or internal QA submission package.

Frequently asked questions

How many participants do we need for cognitive debriefing?
There is no fixed number. ISPOR and FDA guidance call for fit-for-purpose samples, typically 5–8 participants per language. Our standard package covers 5 participants per country, with leaner designs for rare or hard-to-reach populations.

Do we need to involve the instrument developer?
Usually, yes. Most COAs are copyrighted and developers often need to approve translations. We can liaise with them on your behalf.

Can linguistic validation replace psychometric validation?
No. Linguistic validation supports content validity and conceptual equivalence; psychometric validation requires a separate quantitative study.

Can the report be used in regulatory submissions?
Yes. Our LV reports are designed for inclusion in submission packages for EMA, FDA and MHRA.
